A guide for pediatricians, dentists, speech pathologists, ENTs, sleep physicians, and other healthcare providers.

We welcome referrals from providers across Rhode Island and New England. Whether you're managing a child with airway concerns or an adult patient who needs orthodontic treatment before restorative work, we're here to collaborate. Dr. Lavigne, Dr. Cardarelli, and their team communicate closely with referring providers throughout the course of treatment.

Pediatric Referrals

The American Association of Orthodontists recommends that every child be evaluated by an orthodontist by age seven. Early evaluation allows us to identify developing problems and intervene at the most effective time — often reducing the need for more complex treatment later.

Please refer your pediatric patients to us for any of the following:

  • Age 7 evaluationRoutine orthodontic screening as recommended by the AAO. We will evaluate a child as young as 3.
  • CrossbiteRefer as soon as a posterior or anterior crossbite is identified, regardless of age
  • Deep biteEarly intervention can promote forward growth of the jaws and prevent or minimize tooth wear issues
  • CrowdingEarly assessment allows for growth guidance before all adult teeth erupt
  • Breathing & airway concernsWe are an integral part of a collaborative airway team. We work alongside ENTs, sleep physicians, myofunctional therapists, and other specialists to evaluate and address airway dysfunction in growing patients
  • Signs of sleep-disordered breathingIncluding snoring or noisy sleep, tooth grinding (bruxism), bedwetting after potty training, mouth-breathing, dental crowding, and hyperactivity
  • Speech concernsWhen speech issues persist despite speech therapy intervention, an orthodontic evaluation may identify contributing structural factors
  • Tooth grinding (bruxism)Refer as soon as grinding is identified

Adult Referrals

Orthodontic treatment plays an important role in comprehensive adult dental care. We work closely with general dentists, prosthodontists, oral surgeons, periodontists, and other specialists to ensure ideal outcomes for complex restorative and functional cases.

Please refer your adult patients to us for any of the following:

  • Pre-implant orthodonticsAn orthodontic consultation is recommended before any implant is placed to ensure proper spacing and occlusal alignment
  • Anterior tooth wearWhen incisal wear is identified, a bite evaluation should precede any restorative intervention
  • Pre-restorative bite optimizationIf the bite is not ideal before full-coverage restorations are completed, orthodontic treatment first will protect the longevity of the restorative work. We ask that patients be placed in long-term temporary restorations during orthodontic treatment, with final restorations placed after the bite is idealized
  • Canine wear or non-Class I canine occlusionRefer when the patient is not in Class I canine occlusion or shows wear on the canines
  • Adult airway concernsWe treat adult airway issues using the same collaborative, team-based model as with pediatric patients. Orthodontic interventions may include adult palate expansion or coordination with oral surgeons for jaw repositioning surgery

About the Doctors

Dr. Courtney Lavigne is Rhode Island's only board-certified orthodontist and Accredited Member of the American Academy of Cosmetic Dentistry (AAACD) — a distinction held by fewer than 1% of dentists worldwide. She has advanced training in airway health, early intervention, and complex restorative orthodontics, and lectures nationally and internationally. She has a true passion for early intervention to set children up for better growth to avoid complex needs later in life.

Dr. Lauren Cardarelli earned her DMD, Orthodontic Certificate, and Master of Dental Science from the University of Connecticut, where she served as Chief Resident. She is experienced across the full range of orthodontic treatments including clear aligners, customized braces, and advanced appliances including MARPE. Dr. Cardarelli's research has earned multiple national awards in 3D imaging and AI-assisted treatment planning.
